We require that all of our dog families complete all four of the following forms to ensure the safety and happiness of all of the dogs at Dog Play & Stay Resort. The Application form can be completed online, or printed and turned in. The Emergency Medical Care Release, Release of liability and the Veterinary form require signatures. You can be completed online for all forms, or printed and filled out
completely and turned in person.
